Jowls refer to the sagging or drooping of skin and soft tissue along the jawline. They develop when structural support within the face weakens, causing tissue from the cheeks to descend downward. Melanin-rich skin often retains collagen slightly longer than lighter skin tones, meaning visible sagging may appear later. Sagging is not simply about loose skin. It is a multi-layered structural process involving: bone resorption (loss of underlying support), fat pad descent, ligament laxity, collagen and elastin decline. 

As collagen levels reduce, the skin becomes thinner and less elastic. At the same time, deeper fat compartments shift downward due to gravity and ligament weakening. The combination of these changes leads to heaviness in the lower face and the formation of jowls. Sun exposure accelerates collagen breakdown, while smoking and chronic stress can further weaken skin integrity.

There is no single “quick fix” for sagging. Successful treatment focuses on restoring support and stimulating collagen production. Strategic placement of dermal fillers in the cheeks, jawline, and chin can restore lost support and create a lifting effect. By enhancing mid-face projection and strengthening the jawline contour, the lower face appears more defined. Rather than filling the jowl directly, treatment often focuses on structural areas to rebalance the face. Results are subtle and natural when performed with anatomical precision.

Microneedling and other collagen-stimulating treatments help improve skin firmness over time. These procedures deliver controlled injury into the dermis, encouraging new collagen production and gradual tightening. They are particularly effective for early to moderate sagging and can be combined with injectable treatments for enhanced results.

In selected cases, targeted anti-wrinkle injections in the lower face and neck can reduce downward pull from certain muscles. This approach, often referred to as a “Nefertiti lift,” subtly enhances jawline definition by relaxing depressor muscles. Not all patients are suitable, and careful assessment is required.

Sun damage is one of the strongest risk factors for worsening fine lines and wrinkles. Protecting yourself with a hydrating SPF 50 daily, year-round is essential (even if you are indoors in a room with windows!). Without wearing adequate sun protection, any treatments we offer will be of minimal benefit to you in the long term and may affect the longevity of your results.

Stopping smoking, improving your oral fluid intake and following a good skincare routine will also help improve the appearance of fine lines and wrinkles.

Dermal fillers typically last between 9 and 18 months depending on product type and individual metabolism. Collagen-stimulating treatments produce gradual improvements over three to six months, with maintenance sessions recommended annually. Lifestyle factors, including sun protection and skincare, significantly influence longevity.

Non-surgical treatments can significantly improve mild to moderate jowls, but severe sagging may require surgical intervention for dramatic correction.

One of the risks associated with dermal fillers is migration which often occurs as a result of long term use of filler. As a result, we generally tend to avoid treating young clients.  

When placed correctly and conservatively, filler restores balance and definition without heaviness. Overfilling is avoided through careful planning.
